A routine medical transport mission ended in tragedy over the weekend as a helicopter crashed into the sea in southwestern Japan, claiming the lives of a patient, her doctor, and a caretaker.

According to the Japan Coast Guard, the medical chopper was en route from an airport in Nagasaki Prefecture to a hospital in Fukuoka when it plunged into the sea. The crash triggered an extensive rescue operation involving two aircraft and three patrol ships.

Three of the six people on board—pilot Hiroshi Hamada, 66; helicopter mechanic Kazuto Yoshitake; and 28-year-old nurse Sakura Kunitake—were rescued after being found clinging to inflatable lifesavers in the frigid waters. All three were conscious but suffered hypothermia and were rushed for medical treatment, a Coast Guard official told The Associated Press.

Tragically, the remaining three passengers were not so fortunate. Their bodies were later recovered by a Japan Air Self-Defense Force helicopter. The deceased include Dr. Kei Arakawa, 34; Mitsuki Motoishi, 86, who was the patient being transported; and her 68-year-old caretaker, Kazuyoshi Motoishi.

Initial confusion surrounded the spelling of mechanic Yoshitake’s first name, but officials later corrected the error.

Authorities have launched an investigation into the cause of the crash, which remains unknown as of Monday. The Japan Coast Guard continues to analyze evidence and assess the circumstances that led to the fatal accident.
